Parent/Guardian Registration, Payment, and Liability Agreement

I confirm that I am the parent or legal guardian of the player or participant being registered, or that I have permission from the parent/legal guardian to complete this registration.

I am authorized to provide the player's or participant's information to the Organization.

I confirm that the information submitted in this registration is true, accurate, and complete to the best of my knowledge.

I understand that inaccurate or incomplete information may affect registration, eligibility, team placement, group placement, communication, scheduling, or participation.

I consent to the Organization collecting, using, storing, and sharing registration information as needed for Organization operations, including registration processing, team/division/group/program placement, coach/admin communication, scheduling, rosters, payment confirmation, Organization records, and safety, eligibility, and administrative purposes.

I understand that limited player/participant and Organization information may be displayed publicly on the website or in Organization materials, depending on Organization settings and administrator decisions.

This may include the player's or participant's name, division, team, group, roster placement, schedule, game/event information, and similar Organization-related information.

I understand that parent/guardian contact information is not intended to be publicly displayed unless the Organization specifically chooses to publish contact information for a coach, official, administrator, board member, volunteer, or other public Organization role.

This registration is intended to be completed by a parent or legal guardian, not directly by a child without parent/guardian involvement.

By submitting this registration, I consent to the Organization collecting and using the child's information for Organization-related purposes.

If payment is required, I understand that payment may be processed by a third-party payment processor.

I authorize the registration payment shown during checkout and understand that the website does not intentionally store full credit card numbers, CVV codes, or sensitive payment card data.

I understand that refunds, late fees, payment disputes, registration approval, eligibility, division placement, team placement, group placement, and participation decisions are handled by the Organization according to Organization rules and policies.

The website provider is not responsible for Organization decisions, refund decisions, eligibility decisions, payment processor issues, or user-submitted errors.

I understand that participation in sports, practices, games, tournaments, events, programs, volunteer activities, and related activities may involve risk of injury, property damage, illness, or other harm.

I understand that this website is only used for registration and Organization administration. It does not replace Organization safety rules, insurance requirements, waivers, coach supervision, medical judgment, or emergency services.

If the Organization has a separate athletic waiver, medical release, concussion form, code of conduct, photo release, volunteer agreement, or participation agreement, I understand that those documents may also apply.

I understand that registration does not guarantee placement on a specific team, with a specific coach, in a specific division, in a specific group, at a specific location, or on a specific schedule unless the Organization separately confirms it.

Schedules, teams, divisions, groups, locations, and game/event details may change.

I consent to receiving Organization-related communications using the contact information I provide, including email, phone, or other communication methods used by the Organization.

I understand that message delivery is not guaranteed and that I am responsible for checking the website or contacting the Organization for important updates.

I understand that the Organization may keep registration records during the active season/program and may retain past records for Organization administration, accounting, disputes, or historical reference.

I understand that the Organization may later archive a season/program by removing or redacting unnecessary personal information while keeping limited historical information such as player/participant name, team, division, group, season/program, schedule, or public Organization records.

I understand that permanent deletion, when used by the Organization, is intended to remove records from the active website database and may be irreversible.

I understand that reasonable efforts may be made to keep the website available and secure, but no website or online system can be guaranteed to be uninterrupted, error-free, or completely secure.
